Peristalsis is a complex mechanism of rhythmic contraction that propagates like a wave along the gastrointestinal tract, increasing in intensity, peaking in the small intestine, and diminishing in the colon.
The peristaltic wave begins immediately after swallowing in the upper esophagus,
guiding food:

a. from the esophagus to the stomach, where it is broken down by gastric juices;

b. to the small intestine, where digestion is completed and most nutrients are absorbed;

c. to the large intestine, where it is transformed into fecal matter.

At the rectal sphincter, along with the peristaltic wave arriving from the preceding intestinal segment, the voluntary intestinal musculature acts synergistically to consciously inhibit or allow stool evacuation. In addition to the intestinal wall muscles, defecation also involves the diaphragm and abdominal muscles, which contract simultaneously to generate sufficient pressure for expulsion.

Peristalsis varies in speed depending on the presence of food in the stomach and the quantity and type of material within the gastrointestinal tract. It is influenced by stress (via the sympathetic and parasympathetic autonomic nervous system), hormones such as serotonin, acute or chronic inflammation, lifestyle, physical activity, and the composition and balance of the gut microbiota.

Changes in peristaltic contraction speed are crucial in determining transit alterations and various disorders, including constipation or diarrhea, cramps, bloating, abdominal pain, gas, flatulence, and malabsorption.
Movement stimulates intestinal peristalsis, improving transit speed and efficiency, promoting beneficial bacteria growth and microbial diversity, and reducing inflammation.
